Insurance & Liability & Social Media Waiver

Insurance Waiver *

Winnipeg Punjabi Arts Academy does not carry medical insurance for its students. It is required that all students be covered by Canada's universal health care plan or other family insurance policies. If injury occurs, it is understood that the student's own policy is the source of reimbursement. For students under 18, the parent or guardian accepts this responsibility on behalf of their child. For adult students, this responsibility is accepted personally. By checking this box I confirm that the student is covered under an appropriate insurance plan.

Liability Waiver *

I hereby agree to release and hold harmless Winnipeg Punjabi Arts Academy, its officers, employees, volunteers, committees, and boards from and against any and all liability, loss, damages, claims, or actions (including costs and attorney's fees) for bodily injury and/or property damage to myself or the student I am registering, to the extent permissible by law. This indemnification and hold harmless agreement shall include indemnity against all costs, expenses, and liabilities incurred in connection with any such claim or proceeding and in defense thereof. I voluntarily agree to this release and hereby give permission to Winnipeg Punjabi Arts Academy for emergency transportation and/or treatment in the event of illness or injury to the student. I hereby accept responsibility for the payment of any emergency transportation and/or treatment. I further certify that the student is in good physical condition and has no medical or physical conditions that would restrict participation in this activity or program, and if the student does have any medical or physical conditions I have notified Winnipeg Punjabi Arts Academy accordingly. Furthermore, any damages to property owned by Winnipeg Punjabi Arts Academy (including props, costumes, and uniforms) while in use by the student will be reimbursed by the parent, guardian, or adult student if the property was used without the discretion of Winnipeg Punjabi Arts Academy. For students under 18 this waiver is signed by the parent or guardian on behalf of their child. For adult students this waiver is signed personally. By checking this box I confirm my agreement to this liability waiver.

Social Media Waiver *

I hereby authorize any images or video footage taken of the student, in whole or in part, individually or in conjunction with other images and video footage, to be displayed on all Winnipeg Punjabi Arts Academy social media platforms (Instagram, Facebook, website, and WhatsApp) and any other official channels, and to be used for media purposes including promotional presentations and marketing campaigns. I also authorize the display and use of any media material created by the student within Winnipeg Punjabi Arts Academy. I waive any rights to privacy and compensation in connection with such use of the student's name and likeness, including rights to written copy that may be created in connection with video production, editing, and promotion. For students under 18 this waiver is signed by the parent or guardian on behalf of their child. For adult students this waiver is signed personally. By checking this box I confirm my agreement to this social media waiver.

I understand and agree that all semester fees are non-refundable. In the event Winnipeg Punjabi Arts Academy is unable to offer a registered session due to insufficient enrollment, a full refund of $210.00 will be issued to the original payment method within 5–10 business days.
